James Van Der Beek and Heather McComb had no children together during their almost decade-long relationship. However, James loved children and wanted to be a father. A month after his split from Heather, he married business consultant, Kimberly Van Der Beek (maiden surname Brook) in August 2010. The following month, the couple welcomed their first child, a daughter whom they named Olivia Van Der Beek. Following Olivia's birth, her parents welcomed five more children, named Joshua, Annabel, Emilia, Gwen, and Jeremiah Van Der Beek. It was after the birth of the sixth baby in 2021 that James decided to undergo a vasectomy procedure.

In an interview with People, James Van Der Beek jovially revealed that they "really sucked at not getting pregnant." He noted that five of their six children were unplanned surprises. He said, "We love having kids, but we didn't really plan it out. It just happened." According to him, a vasectomy was the only way to prevent Kimberly from getting pregnant again. He underwent the procedure in 2023, a few years before being diagnosed with stage 3 colorectal cancer. Regarding his vasectomy, the then-47-year-old James jokingly stated:

"I figured the only way we're going to stop this is if we have some medical intervention. I said [to my kids], 'Listen, guys, you know daddy has some surgery on his private parts, so just be really careful when you run up to me not to bump into anything.'"
