For her first weigh-in with Dr. Younan Nowzaradan (aka Dr. Now), Charity Pierce needed a stretcher and a medical team to transport her. Her first weigh-in turned into an alarming situation when she tipped the scales at 778 pounds. Dr Now, right at that moment, made a decision and admitted her to the hospital to "avoid a life-threatening situation." Thereafter, she was kept on a 1,000-calorie diet set by a team of medical professionals.

Eventually Charity Pierce was able to lose 40 pounds while in the hospital. She weighed 729 pounds by the time she was discharged. Dr Now gave her a goal of losing another 40 pounds in order to qualify for weight-loss surgery. However, when she came for her next check-in, Charity had only lost 18 pounds, weighing 709 pounds. Despite not meeting the goal set by Dr Now, he approved the surgery pending more weight loss while hospitalised again; she got down to 650 pounds pre-surgery.

Once Charity Pierce lost the set weight while under supervision of medical professionals, Dr Now performed the gastric bypass surgery, which was a success. Dr Now gave her a goal of losing another 50 pounds. However, her life was faced with yet another hurder soon after. Tragically, her mother died six months into her weight loss journey, leading Charity to slip back into unhealthy eating habits as a coping mechanism.

She lost an additional 57 pounds after masses on her legs were removed during the procedure. Charity weighed 496 pounds when she was discharged from the hospital at the end of the episode. During Season 3, Charity reflected on feeling free and stated:

"I just feel like I'm not stuck anymore. Everything is a possibility now. It makes me feel like I'm starting a new life."
